The Booth Level Officers (BLOs) are responsible for distributing and later collecting the SIR enumeration forms from the electors. One can also fill out the form online by logging in to 'voters.eci.gov.in'. However, if a person doesn't receive the form, they can contact their BLO through the 'Book a Call with BLO' option on the same website.
