ISRO Chairman V Narayanan claimed that although SpaceX and Axiom Space engineers observed an oxygen leak in the former's rocket, they were under the impression that it was minor and the Axiom-4 mission could be launched. It was after the Indian team's insistence that the rocket was thoroughly checked, and a crack in the fuel line was discovered and fixed.
