An initial inquiry has blamed the Bilaspur-bound passenger train’s crew for crash near Lalkhadan, saying they "failed to control the train at danger signal". The South East Central Railway report said the crew committed a 'Signal Passed at Danger' (SPAD) error, causing the train to hit a stationary goods train. Eleven people, including the loco pilot, were killed.
