Enumeration form can be found by visiting Election Commission’s website. Select your state, enter your EPIC number, then form will appear. If EPIC number is not linked to your mobile number, then the form cannot be filled up. However, ECI is providing opportunity to link mobile and EPIC numbers. After completing that process, enumeration form can be filled up online.
